Please remember to bring:

• Your completed medical history form
• Your current medication form listing dosage and frequency, including over-the-counter and herbal supplements
• Insurance card(s), co-pay and picture ID
• Copies of recent tests, imaging reports (x-rays, CAT scans), past procedure reports or anything you want your provider to review during your appointment

If you are having a procedure, please remember to:

• Review your preparation instructions
• Bring a licensed driver who can wait during your procedure (approximately two hours)
• Wear loose, comfortable clothing
• Bring your current medication and allergy list.
• Leave valuables at home (except insurance card(s) and deposit, if required)

If you are a new patient and will need to fill out paperwork, please arrive 30 minutes early.
If you are a returning patient, please arrive 15 minutes early.
