Your bill for our services will come from Dr. Carl Jones/Athena Health. We will gladly work with you to set up payment plans options. Financial responsibility rests with the patient for deductibles, co-insurance, and non-covered services. We accept Visa, Mastercard and Discover.

Missed Appointments Policy: If you miss your appointment, there will be a fee of $50. If you miss an appointment three times, you will be discharged from the practice.

Missed Procedure Policy: If you miss your procedure, you will be charged a fee of $250. If you must reschedule a procedure please contact us at least 24 hours in advance.
